Iatrogenic esophageal perforation with mediastinitis, multidisciplinary management: a case report

Acute mediastinitis is a low incidence pathology, but it associated with high mortality rate. Iatrogenic lesions are the most common cause of secondary to esophageal perforation. Early diagnosis and surgical treatment important factors in patients this condition. 83-year-old female patient, history left saphenectomy due venous insufficiency difficult intubation two days prior emergency consultation. She was admitted room dyspnea, hemoptysis, chest pain, right hemifacial edema. A tomography performed findings suggestive perforation or airway injury. Later, an upper digestive tract study, extravasation contrast medium documented posterolateral wall cervical esophagus collection middle mediastinum. The patient taken for drainage by thoracoscopy presence confirmed intraoperative endoscopy. Esophageal repair performed, prevertebral space superior mediastinum longitudinal cervicotomy. Postoperatively, she received antibiotic enteral nutritional support nasojejunal tube. Low output fistula esophagus, organized drain, documented, which closed after 4 weeks conservative management. very rare entity mortality. prognostic factor
